Closing gaps in access to care
What it is
The Community Impact Fund is a grant program from ww5 that supports community-based nonprofit organizations based in the United States working to close gaps in access to care. This is part of ww5' Champions in Care initiative — independently administered by Corporate Giving.
Why it matters
Even with major advances in medicine, communities across the United States face persistent barriers to accessing quality healthcare, which may be based on geographic, cultural, language, environmental, social and economic factors. This fund exists to support trusted, locally embedded community-based organizations working to close these gaps for people who are medically underserved.
Where we focus
Grants support programs across ww5' core therapeutic areas. Please see specific areas of interest.
What we look for
Programs designed to make measurable differences in the lives of people who are medically underserved in our priority focus areas.
Getting people into and through care
Helping people who are medically underserved to navigate the process of getting care and overcome significant barriers to continuing care.
Catching disease early
Driving earlier screening and detection with the goal of reducing the burden of disease and improving outcomes.
Building knowledge and support
Empowering informed, engaged patients, caregivers and communities.
Addressing barriers to care
Tackling the root causes that shape health outcomes.
Environmental & health
Understanding how environmental conditions such as extreme heat, poor air quality, flooding, drought and severe weather can have direct and indirect effects on health, access to care and the resilience of communities
